SW Locums are seeking an experienced and motivated Mental Health Social Worker to join the South Locality Adult Social Care Mental Health Team. This is an opportunity to work within a dynamic, multi-disciplinary environment, delivering high-quality social care support to adults experiencing mental ill health across our community.

Must be available for immediate start

Hybrid (office-based with one early morning or late evening outreach shift per month)

You will play a key role in supporting residents to achieve positive outcomes, working closely with health partners, community services and internal teams. The role requires someone who is confident, strengths-based in their practice, and able to manage a varied and fast-paced caseload.

Key Responsibilities

The successful candidate will be responsible for:

  • Participating in the daily duty rota and handling incoming calls and referrals
  • Undertaking Care Act strengths-based assessments
  • Completing Care Act care and support plans and conducting reviews
  • Applying for funding for placements when required
  • Working jointly with health colleagues to develop Section 117 aftercare plans
  • Attending MDT ward rounds and community MDT meetings
  • Planning safe and timely discharges for individuals in acute or psychiatric hospital settings
  • Acting as a Lead Enquiry Officer (LEO) for safeguarding enquiries
  • Carrying out Carers Assessments under the Care Act
  • Undertaking Mental Capacity Act assessments
  • Participating in team meetings and contributing positively to team development What We Expect This Role to Achieve

Qualifications:

  • Degree or Diploma in Social Work or Occupational Therapy.
  • Social Work England Registration.
  • Enhanced DBS (required prior to start).
  • Proven experience in local authority settings and working with vulnerable adults.
  • Knowledge of equipment provision, minor and major adaptations, and relevant legislation.
