SW Locums are looking to find a reliable and conscientious social worker to work on behalf of John Radcliffe Hospital in Oxford. The post is to manage a caseload involving the assessment of clients needs, determining the appropriate support plan/care package and ensuring its implementation to maintain or improve client wellbeing.

Looking for an experienced, HCPC Registered Qualified Social Worker to complete assessments in the community with those discharged on a Reablement pathway.

Assessment to consider eligibility for OCC services


Conduct assessments of clients’ circumstances and issues to determine intervention / referral to the appropriate service.

Accurate identification of clients’ eligible needs.

Relevant responsible Authority is informed according to procedure.

Risk to the health, safety and wellbeing of clients and the public is effectively reduced.

Plan and implement interventions and actions for allocated cases.

Monitor and review cases.

Assist in more complex cases.

Care plans are effectively formulated and delivered.

Provision of agreed intervention is delivered / co-ordinated.

Third sector organisations are utilised where appropriate.

Clients are supported to achieve their identified outcomes.

Support to clients is provided to the required standards and timescales.

Clients assessed needs continue to be evaluated.

Action is taken on any risks identified.

Protection of vulnerable individuals

Implementation of statutory duties where required.

Advice and guidance are sought when required.

Provide professional advice and guidance to colleagues and partner agencies supporting the relevant client group.

Professional advice and interpretation is provided on procedures, policy, legislation, systems, methods etc.

Cases are escalated as appropriate.

Represent the views of the client when appropriate.

Liaise internally and externally and work with colleagues from other agencies to ensure clients are supported. Promote initiatives and share knowledge of best practice.

Clear, accurate information is shared via the most appropriate channel.

Decisions taken are informed by up to date, accurate information and legislation.

Actions taken are appropriate to the known circumstances.

Maintain properly documented case files, records and client information.

Prepare standard reports as required.

All required records / information are accurate, up to date and in the correct format and location.

Clear record of actions, circumstances and decisions are provided,

Reports are prepared correctly.

Information is managed in compliance with legislation and best practice.

Research information to support services to the client group.

Appropriate information sources are identified and used.

Evidence based recommendations are made.

Prepare material and act as witness and/or Officer of the Court when required.
