SW Locums are seeking a Qualified Social Worker to complete work in a community team to hold and effectively manage a complex and varied caseload with appropriate supervision, guidance and support. Ensuring a strength-based conversation tailored to people’s needs underpins all conversations and assessments.

You will be accountable for complex decision making and risk management in relation to caseloads ensuring professional judgement, evidence-based tools, critical reflection and analysis to inform this. There is a requirement to participate in a duty system.

Expectations:

  • You must be able to display previous UK experience to show that reports are up to date, of a high quality and submitted according to appropriate timescales.
  • You will be capable of undertaking mental capacity and best interests’ assessments and ensure the findings are recorded in a timely manner, in line with the Mental Capacity legislations.
  • Lead safeguarding enquiries, ensuring completion within timescales and outcomes recorded and shared with relevant parties, in line with the Care Act 2014.
  • Undertake direct work with people, carers and families in line with interagency safeguarding adults Communicate skilfully and confidently in complex or high-risk situations and escalate to senior management, as appropriate.
  • contribute to the operational delivery of an effective Adult Social Care Service as part of an integrated team responsible for safeguarding, support planning and promoting the wellbeing of adults with care and support needs continuously improving outcomes.

Minimum Requirements:

  • To be a registered Social Worker with Social Work England.
  • Educated to degree level with appropriate professional qualification relevant to the role and specialism (e.g.- CSS/CQSW or DipSw Dip COT or Degree in Occupational Therapy)
  • Knowledge of the Care Act 2014, the Mental Capacity Act 2005 and other relevant legislation.
  • Knowledge of current and proposed Social and Health Care legislation particularly that relating to adults. An appreciation and understanding of current issues in social work practice, social sciences and theory.
  • Excellent knowledge of safeguarding, mental capacity and deprivation of liberty legislation and best practice.

Duties: 

  •  To assist other social workers and SCA’s by offering support and advice within post holders’ area of work.
  • To participate in the duty system.
  • Act as Trusted Assessor providing equipment/minor adaptations and key worker for other allied health professionals (AHP) tasks as appropriate and agreed by team leader/community’s manager.
  • Work/support AHP Trusted Assessors.
  • For specified teams there may be a requirement in order to deliver integrated services with health and where there is a demonstrable need due to the council’s legal duties
    under the Care Act 2014, to work within a seven-day or 8am to 8 pm service.
  • Take responsibility for the professional learning and development of self and others including participating in team meeting and contributing to the development of the team
  • To plan and co-design individual support plans.
  • To ensure that regular monitoring and reviews of support plans are carried out.
  • To be responsible for social work provision including managing own caseload whilst managing the reputation of the Department and Council and complying with national and service procedures and guidance.
  • Hold and effectively manage a complex and varied caseload with appropriate supervision, guidance and support, reflecting the function of the team and Adult Social Care in accordance with policy, procedures, guidance and legislation.
  • Undertake work within legislative frameworks including the preparation of written reports, statements and representations to the judiciary at Court hearings and applications to the Court of Protection.
  • Accountable for complex decision making in relation to caseloads ensuring professional judgement, evidence-based tools, critical reflection and analysis to inform this.
  • Communicate skilfully and confidently in complex or high-risk situations.
  • Model and help colleagues develop communication skills.
  • Attend and represent the department at a range of multi-disciplinary forums and undertaking the role of Chair as required.
  • Undertake direct work with service users, carers and families in line with interagency safeguarding adults’ policy and procedures.
  • To make appropriate assessment and act as care navigator for the individuals presenting with eligible social care needs within appropriate time limits and in collaboration with health and other agencies.
  • To plan relevant responses to meet individual needs in collaboration with carers, advocates and representatives in consultation with health and other agencies.
  • To ensure the delivery within financial resources available of services to meet assessed needs and outcomes.
  • To maintain an effective system for coordination, monitoring and reviewing support plans and service standards, to identify gaps in service or unmet need and advise line manager and commissioners.
  • To work jointly with Social Care Assessors by identifying appropriate tasks or areas of work in agreement with team leader or community’s manager.
  • Social workers who hold an Approve Mental Health Professional (AMHP) qualification to undertake appropriate duties specific to this role. In doing this AMHP’s will work closely with acute, primary and community services. As agreed with the post holder, to participate in the Out of Hours AMHP rota.
  • Social workers who are practice educators will facilitate regular student placements.
  • Undertake capacity and best interests’ assessments.
  • Lead safeguarding investigations ensuring completion within timescales and outcomes recorded and shared with relevant parties.
  • Work co-operatively with colleagues and in partnership within the Council and across multi – agency boundaries.
