SW Locums are looking to appoint a committed, highly motivated, enthusiastic and skilled Band 6, Occupational Therapist to join the integrated multi- disciplinary team in Greenwich.
This post is for an Occupational Therapist with advanced skills and knowledge.
The successful applicant will be involved in collaborative working between Oxleas NHS Foundation Trust, Royal Borough of Greenwich and Lewisham and Greenwich NHS Trust.
The post available is within the Joint Emergency Team which is a multi-professional team consisting of Nurses, Occupational Therapists, Physiotherapists, Social Workers / Social Care Assessors and GP’s.
The team aims to avoid unnecessary presentation to hospital by supporting community patients who are in crisis as well as preventing direct hospital admission from Accident and Emergency.
The team act as a liaison between acute and community settings, by identifying the most appropriate onward referrals and initiating integrated care pathways, ensuring seamless provision of care and on- going case management.
Referrals are received from Health and Social Care professionals, Medical Teams, General Practitioners and Ambulance Services.
Please note – potential candidates will require a significant amount of community experience. Additional experience in acute settings would also be preferred. Responsive, and quick turnover are required within the team, and the expectation would be that any successful applicant would assess a large amounts of clients with a view to providing quick intervention, managing presenting risks, any onward referrals and then closure of said cases.
There will be no long term OT case management.
