SW Locums are excited about this opportunity for a Senior Practitioner within the Adults and Communities team In Hereford.

This role will be working in South Locality Team.

Working hours would be 30 or 37 Potential of mixed approach to work location 2 days in the office 3 remote working

Interviews can be held remotely or in person – Start time anticipated to be 1 week

You will hold a small caseload and you will be supported to make evidence based decisions, working as part of the locality management team to support performance and lead practice.

You will have scope to practice alongside a variety of customer groups and a whole spectrum of people with health and social care needs, such as mental health, learning disability, physical disability, frailty and safeguarding.

You will need a strong commitment to personalised and outcome focussed practice, with a sound knowledge base about adult social work, strong interpersonal and organisational skills, a developed understanding of social care practice, legislation and knowledge of incorporating community assets into service provision.
