SW Locums need the services of a Qualified Social Worker for the Older Adult Mental Health Team North is based in Colchester. This post will be covering both Colchester and Tendring, and the team has an exciting opportunity for a temporary social worker to join them.

The role will involve working with adults aged 65 years and over diagnosed with a functional or organic mental illness such as dementia; and their families both in the community, and developing discharge pathways from inpatient wards in accordance with the Mental Health Act 1983 and Care Act 2014 principles.

This is a dynamic role promoting innovate person centered and partnership working with adults, their informal carers and in collaboration with other statutory, and non-statutory services to support and promote their mental health, well-being and outcomes.  An essential part of the role is to challenge stigma, discrimination and consider least restrictive approaches for adults and their situations through holistic assessments and considering positive risk management to promote outcomes and recovery.

The role is a Monday to Friday 9am to 5.30pm.

The purpose of this job is to raise or contribute to discussions of issues and proposals by developing and leading integrated social work practice within the relevant service area.

Core Accountabilities

  • Undertake comprehensive needs-led assessments under the Care Act 2014 and prepare, implementing and the monitoring of care plans
  • Working with service users and their families/carers
  • Attending and participating effectively in supervision by team leader/senior practitioner on regular basis in accordance with Directorate policies
  • Co-working effectively with other professionals including attending multidisciplinary case discussions and team meetings
  • Recording information accurately in line with ECC and EPUT policies
  • Expected to be committed to continuous workplace learning through organisations and attendance at social care forums and meetings
  • Any other duties as may reasonably be required
  • Ensure social care services are commissioned with best value and personalised outcomes in mind and ensure that social care funded packages are reviewed on a regular basis in a robust way.
  • To contribute to the commissioning and provision of effective after-care services, including those required under s117 MHA, Care Act 2014 and CP.

In line with government legislation, all colleagues who work in a CQC registered care home or will need to attend such a setting as part of their role must be fully vaccinated against COVID-19.
